•        facilitate communicationbetween the patient and the interdisciplinary team.

•        develop and maintain atherapeutic relationship by providing education and promoting self-advocacy andindependence.

•        utilize state-of-the-arttechnology to document skilled services provided.

qualifications

•        licensed practical nursewith valid ct lpn license

•        one year of lpn experience;homecare experience preferred.

•        valid driver’s license andreliable transportation required.

•        computer proficiencyrequired.
