vitas healthcare is hiring registered nurses in the west and northwest suburbs of the chicagoland area. the vitas rn is a member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. the vitas rn is designated to provide coordination of care, ensure continuous assessment of each patient's and family's needs and implementation of the interdisciplinary plan of care.

valid registered nurse license for the state of illinois at least 1 year of rn experience within the last 5 years reliable transportation with adequate insurance coverage for driver

responsibilities may include:

patient case management and supervision develops, coordinates, and follows the patient's individualized plan of care appropriately delegates and supervises nursing tasks to hospice aides/homemakers, volunteers, and licensed practical/vocational nurses completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement participates in patient care and team meetings appropriately identifies/utilizes resources completes documentation, shift reports, and provides effective communication other duties as required and assigned
