job responsibilities:
to maintain meaningful communication with the resident’s care providers
assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
coordinate and supervise clinical care for residents
clear and concise documentation in accordance with facility requirements
qualifications/requirements:
prior experience as a case manager
or related education.
excellent phone skills and ability to work with families/other care providers
strong organizational skills, passion and compassion for the population
ability to work cooperatively with multidisciplinary team
bachelor's degree is required
