we have an exciting opportunity for you to join the community enhanced rehabilitation team (cert). cert is a new community rehab team within the enhanced rehabilitation and recovery pathway.
comprising of a multi-disciplinary team, cert provides transitional support and mental health rehabilitation focused interventions in the community for adults with severe and enduring mental health difficulties. we work within a recovery approach, undertaking collaborative interventions with service users, families and wider networks within the community. cert offers service users and carers a greater choice over their care and recovery, ensuring interventions are delivered in the least restrictive setting.
