position summary:
under the supervision of the wellness center supervisor, the licensed social worker will be responsible for providing services that promote the social, emotional, mental and spiritual dimensions of wellness for residents.

major duties and responsibilities:
proficiency in computer skills needed for the position

comprehensive understanding of referral resources available for a variety of needs to support the social, emotional, mental and spiritual wellbeing of residents.

follow up on residents who have returned from a higher level of care setting to determine if supportive resources will be received to the benefit of the resident.

provide engaged active listening skills for residents, providing counsel and encouragement in a professional and objective manner while maintaining and empathetic approach.

engage families in dialogue to gain understanding of resident perspectives as well as to educate families on resident transitions.

collaborate with rn team in resident assessments administers psychological/emotional assessment instruments and develops treatment plan based on results.

coordinates with social, medical, wellness, and spiritual agencies and resources, healthcare providers, and community outreach programs to provide residents with needed services.

document actions in the resident medical record.

facilitates spirit lifter program.

coordinates other support groups through benevilla and/or other resources that would benefit the overall health and wellbeing of residents served.

contact mpoa after a resident passes to offer emotional support as well as information regarding quarterly memorials, and other supportive services available. this duty would be for residents across the continuum of living areas at royal oaks.

interacts courteously and appropriately with residents, family members, visitors, and other staff members under a variety of conditions and circumstances.

collaborates within the wellfit department staff on fitness/wellness projects, for the greater good of the staff members and senior population at royal oaks, within the scope knowledge, skills and abilities.

maintains the comfort, privacy, and dignity of each resident and staff member in the delivery of services. interacts with residents and staff members in a manner that displays warmth and promotes a caring environment.

full understanding of residents’ rights, including the right to be free of restraints and free of abuse. is responsible for promptly reporting to the director of health services, or the president and chief executive officer (ceo) incidents or evidence of resident abuse or a violation of residents’ rights.

observes residents for changes in condition or behavior and promptly reports these changes to the supervisor, nurse, or community safety and emergency services department.

assists in maintaining a safe, neat, and clean environment. reports all deficiencies to the appropriate person, such as equipment problems, need for repair or special needs of residents.

ensures facility and department policies, procedures, and guidelines, as well as local and state regulations, are met and followed.

performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.

is committed to keeping confidential any sensitive information.

strive for innovative ways to improve the daily functions within the wellfit department.

attends and participates in training activities, meetings, seminars and assist other areas when needed.

performs any other function as trained and qualified to perform at the direction of the supervisor.

knowledge and skills:
able to gather data, compile information, and prepare reports
skilled in database management
demonstrated ability to maintain confidentiality
skilled in organizing resources and establishing priorities
ability to work independently, exercising sound judgment
demonstrated values of teamwork and integrity
ability to read, understand, follow and enforce safety procedures
computer proficiency- microsoft word/excel, outlook and internet
organized, able to multi-task, think creatively and be able to adapt to different situations.
education and experience:
bachelors degree in social work, family counseling or similar degree program required
preferably one year of experience in serving seniors
strong computer and time management skills
competency in the position through continuing education.
must obtain and maintain the following:
cpr and first aid certification with american heart association within 30 days of hire. can complete at royal oaks after hire.
fingerprint clearance card apply within 10 days of hire. can complete at royal oaks after hire.
proof of freedom of tb upon hire and annually. can complete at r
communication skills:
- has clear speaking voice, must be fluent in english.
- ability to speak, work and interface with all levels of co-workers, residents and guests.
- ability to write routine reports and correspondence.
- demonstrates excellent telephone and customer service etiquette.
