this role is dedicated to enhancing medical behavioral integration activities in medical primary care settings, as well as providing telephonic care management. the focus is to support implementation of optum’s behavioral health virtual collaborative care model. this position will support primary care providers prescribing behavioral health medications by connecting them to a consulting psychiatrist. as a care manager, you will be responsible for case management of mental health and substance use disorder services. you'll have a direct impact on the lives of our members as you recommend and manage the appropriate levels of care throughout the entire treatment plan. this program will directly improve behavioral health care within the medical setting and coordinating with behavioral health specialty care

primary responsibilities:

participate and collaborate with customers, stakeholders, and providers on various projects and initiatives serve as a key resource on behavioral health for primary care provider and our customer  provide guidance on best practices in care coordination, linking patients to health plan and community resources, and self-management tools collaborate with primary care providers & psychiatrists  provide support to members in the program through telephonic care coordination including assessments, psychoeducation, resource referrals, and translating medication questions and concerns to the primary care provider educate members on their behavioral health benefits and coordinate services/care  leverage data while implementing measurement-based care  serve as the liaison between primary care practices and optum behavioral health to address questions and resolve issues  complete member assessments and individualized care plans to address members’ needs telephonic visits with members to determine/coordinate treatment or resources to address members’ complex needs managed transitions between care settings and/or discharge planning to ensure appropriate services/resources are in place for quality outcomes manage behavioral care coordination for all complexity levels make outreach calls to members and/or providers for coordination of behavioral and medical services

required qualifications:

master's or doctoral degree in psychology, social work, counseling, marriage and family counseling, or related field active, unrestricted, independent behavioral health license in the state of minnesota (i.e., licsw, lmft, lp, lpc, or lpcc)  3+ years in a clinical/management role or equivalent managed care experience experience analyzing and interpreting data, and developing, distributing, and explaining reports on service delivery live within a commutable distance of our eden prairie, mn office location full covid-19 vaccination is an essential requirement of this role. unitedhealth group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance

preferred qualifications:
