to provide a variety of patient care and support activities fulfilling the needs of the supportive care program in both office/clerical duties, and clinical duties as necessary in clinic or patient home settings. 

primary responsibilities:

performs all nursing duties within the scope of a licensed vocational nurse or licensed practical nurse as required by state of employment and within wellmed-defined scope of practice records patient care documentation in the medical record accurately and timely reviews the patient record, chart, reports (including laboratory and x-ray), and other pertinent information for each patient prior to being seen by the provider and reports relevant information to the provider 100% of the time coordinates patient care as directed by provider, company standards and policies processes appropriate documents in organized and accurate fashion respects patient confidentiality organizes housecall bags and maintains equipment; responsible for ordering equipment as necessary maintains quality and infection control standards maintains required licensure & certifications (lvn or lpn & basic life support) answers phones and schedules appointments based on best geographic locations to minimize drive time between patients schedules family meetings when appropriate ensures proper communication between primary care physician and supportive care team intakes patient phone calls to the appropriate destination, ensuring that urgent calls are expedited to the provider prepares patient chart for each provider’s daily visits: re-orders patient visits as needed to maintain routing efficiency reviews patient gaps in care; completes necessary measures and/or prepares provider to complete patient charts updated with labs, data, consult notes, hospital records, etc. assures consents are in chart when appropriate manages medical records according to established company guidelines 16. performs independent or partnered patient home visits as necessary performs independent or partnered patient home visits as necessary organizes exam and treatment rooms, documents and assists in procedures as directed by provider collects samples (labs) and ensures proper labeling/dissemination in both home and clinic settings fulfills provider orders including medication refill requests performs all other related duties as assigned

full-time field base position with some in office requirements: this position requires traveling around the san antonio, tx and counties areas supporting wellmed patients

must have reliable transportation that will enable you to travel to client and/or patient sites within a designated area. “mileage reimbursement available”

$1500 sign on bonus for external candidates

required qualifications:

high school graduate or general educational development (ged) equivalent current unrestricted licensed vocational nurse or licensed practical nurse in applicable state of employment current bls certification 3+ years of related lvn or lpn clinical experience 1+ years of phlebotomy experience computer literacy knowledge of medical terminology knowledge of medical office procedures ability to sort and file materials correctly by alphabetic or numeric systems reliable transportation this position requires tuberculosis screening as well as proof of immunity to measles, mumps, rubella, varicella, tetanus, diphtheria, and pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease

preferred qualifications:

3+ years of lvn or lpn experience in a medical, home or hospice setting knowledge/experience in hospice, palliative care, chronic care and end-of-life patient management knowledge of international statistical classification of disease (icd-10) and current procedural terminology (cpt) coding intravenous certification good communication and customer service skills with ability to establish and maintain effective working relationships ability to react calmly and effectively in emergency situations

physical and mental requirements:
