facilitating the patient's care throughout the health care fielddevelop the plan of careprovide direct care management services to clients living with hiv/aidsmaking changes to care plancoordinate resident care with appropriate staffcollaborate and coordinate care with health care professionals and social service providersuse registry and other care plan information to inform care team members of care plan implementation required for each patientexecute assisted living resident care budget for communityprovide continuous care management across community, hospital and nursing home care settingsmonitor and reassess care plansbuild alliances with other providers of caresupport the delivery of high quality care across the health care systemperform assessments, care coordination, care planning, and facilitation of servicesidentify and arrange for care services needed to implement the plan of carecare as patients transition through different levels of care and care settings per the care transitions protocolevaluate the effectiveness of the care plan in meeting established care goalsoversee services between patient and extended care team providers to ensure that integrated care plan is fully implementeddevelop and implement a care planfacilitate patient referrals to specialty caresupport the members' overall care management plan
