reporting to the lead social worker is responsible to work as a member of the case management team. the social worker will have ongoing interaction with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations.
social workers educate staff regarding effective allocation of the hospital's resources while encouraging provision of high-quality patient care.
must remain cognizant of regulations and policies of all review organizations relating to billing of the fiscal intermediaries’ coordinates while monitoring and evaluating options and services to meet an individual’s health needs.
provide a full range of social work services in collaboration, and communication with interdisciplinary team.
the social worker will assess, plan, implement, coordinate, monitor, and evaluate options for a safe and timely discharge.
required skills:
ability to work with diverse patient and staff populations with knowledge and sensitivity to cultural and spiritual influences impacting on patient care.
exhibit a positive attitude
ability to document discharge plans in a clear and concise manner
knowledge of state and federal programs that provide medical care and financial support to individuals and community resources
ability to work independently, with little direction
knowledge of the continuum of care and utilization processes
perform other functions as needed
required experience:
master’s in social work
valid new jersey lsw
minimum of two (2) years’ experience working in a hospital setting
two (2) years’ experience working in palliative or hospice care
