an active member of the interdisciplinary healthcare team this position facilitates internal and external delivery of healthcare services.  the case manager integrates the roles of utilization management, case management and discharge planning. the job objective is to promote a collaborative interdisciplinary care delivery model to achieve optimal clinical and financial outcomes with adherence to regulations and compliance.


perform initial and ongoing discharge screens/evaluations to determine general health behaviors, practices and resources that will influence the patient and family’s abilities to optimally manage chronic conditions.collaborate with health care team to determine extent of functional deficits and areas for potential improvement.assess emotional status and quality of relationships and adequacy of social support systems, identifying new sources of support.prioritizes immediate plan for barriers to discharge including medication availability for use, shelter and appropriate food, primary care physician coordinating care resources accordinglyassures patient /family/caregiver understands and has access to all prescribed post discharge therapies.communicates with patients/families admission to dischargeperforms clinical reviews based on utilization review planmaintains clinical and financial documentationparticipates in performance improvement processes as well as other duties as required by the job

required education: 

high school diploma or equivalent preferred required

two years of college preferred required

associate’s degree preferred required

bachelor’s degree   preferred  required

master's degree preferred required

doctoral degree preferred required


required skills:

ability to understand, communicate, and react effectively to patient and family needs critical thinking work well independently while keeping calm and maintaining customer focus must be able to work collaboratively with internal and external resources for optimal discharge planning

required license/certifications:

current/active north carolina or compact rn license
participates in performance improvement processes as well as other duties as required by the job
