the hills post acute is looking for a motivated social services director to join our team. 
 
we are a skilled nursing facility with decades of dedication and experience serving our residents. 
    
essential functions:

plan, develop, evaluate, and direct the social service programs of this facility.  participate in discharge planning; development and implementation of social care plans and resident programs.delegate authority, responsibility, and accountability to other responsible department personnel.participate in community planning related to the interest of the facility and the services and needs of the residents and family.refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.great customer service with a smileteamwork & a positive can do attitude

education / requirements:

must have either a bachelor’s degree in social work or a bachelor degree in human services, including but not limited to ( sociology, special education, rehabilitation, counseling, psychology).
