staffing for spectrum pod, clinical pharmacist will be responsible for ongoing pharmacy management for members attributed to physician/physician practices participating in unitedhealthcare’s uhc spectrum program. pharmacist will engage with primary care providers (pcps) through pod structure to ensure care coordination/management of a spectrum member pharmacist will work in coordination with the pod care coordinator to address the needs of the members served by the pod. 

note: qualified candidates must live in the baltimore md regional area to perform daily travel expectations of the market region

primary responsibilities: 

accountable for management/direct oversight of care team consisting of a clinical pharmacist, two care coordinators, and a clinical administrative coordinator; hire, train, mentor and evaluate the performance of direct reports to develop a high-performing team conduct comprehensive medication reviews (cmrs) for members identified as high risk, members referred to pod from pcps and the pod care manager. conduct medication reconciliation post-discharge with members discharged from inpatient facilities. collaborate with pod care coordinator to facilitate post-discharge pcp/specialist appointments and 90-day visits on behalf of member. key areas for focus during medication reviews: potential drug related problems, drug to drug interaction, cost-savings opportunities (eg, brand to generic, lower tier alternatives), duplicative or unnecessary drug use. consult and recommend to the pcp alternative pharmacotherapy based on results of cmrs, meet with pcp to discuss the recommended adjustment plan, meet with pcp and member as needed to discuss plan. conduct quarterly targeted medication reviews (tmrs)   monitor pod member medication compliance; consult with members to ensure medication compliance, educate members on importance of medication compliance, benefits of home delivery, etc. identify and address patient specific barriers to medication adherence and disease state management. collaborate with patient’s pcp to overcome barriers and improve adherence and disease state management. provide education to spectrum providers when needed related directly to the spectrum member’s medication management issues.

required qualifications:

doctor of pharmacy (pharmd) degree from an accredited school of pharmacy current licensure in state of maryland 5+ year(s) of experience in a clinical pharmacy setting with direct patient care experience (i.e. managed care organizations, hospitals, clinics, provider offices, etc.) 3+ years of experience communicating with patients, providers, and other healthcare professionals 3+ year(s) of experience managing teams experience with medication therapy management & comprehensive medication reviews in depth knowledge of pharmacotherapy and pharmacy practice ability to prove technical knowledge of microsoft excel; ability to quickly learn clinical applications   ability to work flexible schedule to accommodate business needs ability to travel up to 25% of the time within the baltimore md regional area (qualified candidates must reside in the baltimore md regional area to perform these responsibilities)  ability to provide proof of a valid driver’s license and current auto insurance   full covid-19 vaccination is an essential requirement of this role. candidates located in states that mandate covid-19 booster doses must also comply with those state requirements. unitedhealth group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance
