the licensed social worker is a member of the care management team and responsible for assisting in the management of the continuum of care from admission through discharge for assigned patients. the role reflects appropriate knowledge of social work scope of practice, current state requirements, cms conditions of participation, emtala, patient bill of rights, ab 1203 and other federal or state regulatory agency requirements specific to the social work role. the social work team member will partner with the multi-disciplinary team utilizing best practices, relevant data and compliance with the mission/philosophy, standards, goals and core values of uams: comfort, hope, and healing.
primary elements of the social work role include: assessment of the psychological, financial, and discharge planning needs of the patients and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources for a variety of age-specific populations, including neonatal, pediatric, adolescents, adults, and geriatric.

salary: $30/hour
responsibilities
completes assessment for patients admitted to the hospital. performs discharge planning and aftercare coordination, community referrals and linkages. provides patient and family education and adjustment to illness counseling, grief counseling and crisis intervention. promotes patient and family centered care and assists in coordination of care and services that are appropriate to the specified chronological age, developmental age, social maturity, and cultural background of the patient being treated. serves as a consultant for processing guardianship, power of attorney, health care proxy, and advance directives. may perform other duties as assigned in order to meet the uams core values and the business needs of the department
extra-help positions are required to work three (3) 8-hour shifts a month with a maximum of 1000 hours (1400 during covid) in a 12 month floating period. concurrent employees work one (1) shift a month with a maximum of 1000 hours (1400 during covid) per year in a 12 month floating period.
qualifications
minimum:
master's degree in social work. must be a licensed by an arkansas professional board as a licensed masters social worker or licensed certified social worker (lmsw or lcsw).preferred:
2 years experience in the behavioral health fieldexperience in the acute adult mental health field experience in case management.certified case manager (ccm) or american case management (acm) certification.crisis de-escalation training.this is a safety sensitive position and subject to pre-employment screening (criminal background, drug testing, and/or education verification). a criminal conviction or arrest pending adjudication alone shall not disqualify an applicant except as provided by law. any criminal history will be evaluated in relationship to job responsibilities and business necessity. the information obtained in these reports will be used in a confidential, non-discriminatory manner consistent with state and federal law.
