home recovery care is a care option that allows you to safely receive treatment for your medical condition at home, instead of the hospital. your typical home recovery care lasts 30 days to ensure you are recovering appropriately. our services are available 24/7 to home recovery care patients.

a day in the life:

i oversee the day-to-day operations of the home recovery care team. i have an awesome team of registered nurses who monitor our home recovery care patients. my team coordinates all necessary elements required for a patient to be cared for in their own home instead of a hospital. the goal is to deliver quality care and stellar outcomes while keeping the patient out of the hospital.”

as the care coordinator, you will integrate into the teams at our local health system partner working directly with patients and the clinical teams on the floor of the hospital and telephonically from an office setting. you will serve as the patient’s advocate and work with the patient’s providers to assist develop their personalized care plan.  

 

responsibilities:

coordinates all communication between members of the care team serve as a patient advocate and navigator provides patient education of program including telehealth education and disease specific patient education connect with providers and patients regularly via a telehealth platform coordinate referrals and resources assists in the tracking and prioritization of high-risk patients maintains all required documentation on provider and patient interactions including visits, intake, interventions, patient issues, and any other elements of the patient’s individual plan of care create operational, clinical, and quality reports and presents market updates communicate discharge information identifies trends and hurdles involved in health care system and incorporates solutions monitors the care that the patient receives assist as needed in client and community outreach and critical incident management

 

schedule: monday through friday 8am to 4pm

 

total compensation: $86,000 to $92,000

qualifications:

covid vaccination required* associate of science in nursing; bsn preferred care coordination a current and active rn license 3+ years of acute nursing experience (emergency, med-surg, icu, case management)
