job description:


handle all incoming phone calls in a compassionate and professional manner interview clients to assess need for emergency services, and their need for linkage to other community resources assesses financial need, including expenses, financial resources and eligibility for various public services and supports assesses clients ability to follow through with a plan, including ability to make contacts with other agencies, independently or with assistance assesses adequacy of client documentation for particular funding requirements screens for possible presence of additional problem areas, such as substance abuse that could require additional assessment and follow up, and informs supervisor makes necessary contacts to verify client information in straightforward cases, makes judgments about financial and material assistance to be received from client and through referral to other services records client information and services provided into database in complex or ambiguous cases, makes initial recommendation and obtains decision of supervisor provides education, information and referral to insure successful connection with other appropriate community or entitlement resources counsels and motivates client through the community resource linkage process empowers the client to act for themselves in advocacy and pursuing services, whenever possible when appropriate and necessary, provides direct advocacy for the client through direct contact with other agencies, creditors, etc completes all paperwork and reports in a timely manner as required by supervisors and funding sources participates in assigned meetings and trainings participates in the implementation and ongoing development and continuous improvement ensures immediate and thorough communication to supervisor of program problems and provide recommended resolution to identified problem performs other duties as assigned maintains a positive work atmosphere by acting and communicating in a manner that fosters good relationships with customers, clients, external colleagues and management

requirements:


high school diploma or equivalent (ged) bachelor's degree in the human service field preferred two years of work experience in the human service field valid driver's license and a reliable personal vehicle bilingual: english / spanish preferred knowledge of entitlement programs and program regulations, federal, state and community resources and expertise in navigating these resources has a thorough understanding of all relevant community resources and eligibility or has immediate and ready access to this information customer service savvy with ability to be compassionate excellent written and verbal communication skills strong interpersonal skills proficiency in microsoft office suite (word, excel, outlook) strong data entry skills with the ability to write clearly and proficiently basic math skills ability to make judgements about client assistance strong critical thinking and problem solving skills ability to multi-task ability to rapidly identify potential for physical danger and develop a personal safety strategy maintains confidentiality of all client information and services provided at all times, in accordance with the policy of the client and state and federal laws
