Care coordinator - united kingdom, new milton, hampshire

we are looking for ‘care coordinators’ to join our team.

this role is for 20 hours in our new milton office, and 20 hours within the field covering community visits.

this role is subject to dbs clearance.

· on call commitment (part of a night oncall rota)

key responsibilities:

· working alongside our directors

· supporting our staff within the community

· covering any staff sickness/holidays when necessary

· on call arrangement

· client care plans/reviews

· care needs assessments

· managing the changes in schedules and keeping staff/families informed

· liaising with clients and families and having empathy and understanding of their needs

· liaising with gp practices, district nurses, social workers and other professionals to ensure high standard of care

· providing written reports of all complaints, accidents and incidents in accordance with company policies

· devise and maintain a capacity plan to ensure constant growth for the business

· processing of recruitment including adverts, offers, referencing etc

what are we looking for from our candidate?

care experience with nvq 3 qualification
ideally admin experience but not essential as full training will be provided
excellent computer skills
outstanding telephone manner, attention to detail, ability to work under pressure and be professional at all times
strong adaptability of rostering systems
good time management ability and being able to multi-task
excellent problem solving attitude
reliable and hardworking
team player
ability to remain calm and deal with/support our staff with emergency situations
you will need to be a driver and have use of your car for work purposes
flexibility
ability to work under own initiative and prioritise workload
on call commitment
