an exciting opportunity has arisen for a band 3 health care assistant to join our established community falls service.

we are looking for an individual who has drive and motivation and is looking for a challenge. you must have experience working with older people or those with long term conditions or complex needs.

you will work in partnership with colleagues across traditional care boundaries in the integrated delivery of individualised, safe and effective care that is of high quality. you will want to work within a cycle of continuous improvement and will always be interested in understanding the experiences of our patients and their families.

you will have sound knowledge and skills pertaining to both acute and community settings and will demonstrate an understanding of the whole patient journey. you will demonstrate a caring and compassionate approach to care delivery and you will have excellent interpersonal awareness. you will be keen to develop as an autonomous practitioner and will be confident working collaboratively with colleagues across a wide range of professions and organisations.

you will be a health care assistant in possession of the care certificate and a history of relevant recent and ongoing professional development and you will have a sense of value for inter-professional and work based learning. you will have excellent communication skills and will demonstrate the ability to work well as part of a team.

as from 1st november 2020, it is a requirement of south tyneside and sunderland nhs foundation trust that all successful candidates who require a dbs for the post they have been offered pay for their dbs certificate. the method of payment is a salary deduction from your first month's pay.
