we're looking for bright, hardworking individuals to join our established service in dartford, gravesham, swanley and swale- full and part-time hours available. it's a great opportunity to support adults who need extra help for all sorts of reasons to navigate their way through health, social care and community services; consider what will best improve their health and wellbeing, and make it happen.

navigators bring their own experience to the role. you might have worked in a formal setting, completing assessments and support plans, or directly providing social care or health services.

perhaps you have worked or volunteered in a community setting, or are looking to give something back to your own local community. or maybe you have personal experience of accessing services for yourself, a relative or friend.

it's a really varied role, and you'll spend time visiting people in their own homes or in community settings, speaking to individuals and families. you'll get to know local services, build networks and work in partnership with other agencies, and you will work alongside colleagues in a friendly team so you will feel fully supported in the role. car driver essential.
