as our new lpn, you will:
-provide nursing care for the sweetest senior citizens you have ever met
-administer medications and treatments under physician’s orders
-complete assessments of resident needs
-update service plans
-provide first aid to residents when needed
-respond to and document emergency calls
-be responsible for emr order entry
-share the best practices in nursing education

major duties and responsibilities:
-completes assessments ensuring appropriate level of care is determined and provided.
-completes admit assessment/service plan document within 14 days of a new move in.
-updates service plans per the appropriate required timeframes for supervisory, personal and directed care.
-meets with resident, responsible parties and the al manager for scheduled service plan meetings upon request of the supervisor.
-provides first aid to residents when appropriate and requests physician orders for ongoing treatment or nursing management of a problem that can be safely managed in the assisted living environment.
-responds to emergency calls in the illingworth center as well as to il living environments when needed.
-completes emergency call documentation in the emr for a resident when responding to any emergent situation regardless of the living environment.
-maintains treatment room stock items in an orderly manner, and charges items out per facility protocol using the orbits inventory manager electronic system.
-makes supply request to the purchasing and receiving department when necessary.
-provides nursing education and teaching to residents, recommends and facilitates services as appropriate for a resident as requested by the assisted living managers and under the direction of the wellness center supervisor.
-responsible for entering orders in a timely manner following policy and procedures for emr order entry.
-operates within the scope of practice of licensure per the arizona state board of nursing.
-administer medication, treatments and biological to residents under physician’s orders and in compliance with accepted pharmacy and royal oaks policy and procedures.
-accountability of ensuring proper storing and distributing of medication when applicable.
-follow facility policy and procedure for reporting and documenting any incidents or variances of standard operations.
-ensures that medications and supplies are supplied to maintain adequate levels according to established orders and procedures.
-documentation must be completed per assigned task. progress notes are completed to support the ongoing care and nursing/medical interventions provided to residents.
-provides support and assistance, advice and guidance to unlicensed staff in the assisted living setting, reporting any concerns or needs to the appropriate manager.
-maintains computer competency in systems utilized in the job. correctly query, modify, and create all information in the resident’s electronic record gathered during the shift. maintenance of skills by applying knowledge in the clinical setting is expected.
-ensures that the medical, psychosocial, emotional, functional, cognitive and spiritual needs of residents are addressed and followed up in the service plan in an appropriate and supportive manner.
-maintains the comfort, privacy, and dignity of each resident in the delivery of services. interacts with residents in a manner that displays warmth and promotes a caring environment.
-interacts courteously and appropriately with residents, family members, visitors, and other employees under a variety of conditions and circumstances.
-full understanding of residents’ rights, including the right to be free of restraints and free of abuse. is responsible for promptly reporting to the assisted living manager incidents or evidence of resident abuse or a violation of residents’ rights.
-assists in maintaining a safe, neat, and clean environment. reports all deficiencies to the appropriate person, such as equipment problems, need for repair or special needs of residents.
-makes every effort to protect the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc.
-ensures facility and department policies, procedures, and guidelines, as well as local and state regulations, are met and followed.
-when necessary, lifts, moves, and transports residents, using proper body mechanics or lifting devices.
-performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves and other personal protective equipment where indicated, and proper disposal of soiled materials.
-utilize appropriate techniques for effective assessment, planning, intervention, and evaluation of residents and their needs.
-stays informed of trade and legal developments relevant to the nursing profession. upgrades skills and maintains knowledge of trade/industry advances and new technologies.
-attends and participates in supervisory training activities, seminars, and meetings.
-performs other duties and projects as assigned.

knowledge and skills:
-thorough knowledge of professional nursing practices/theories, techniques and tactics.
-excellent oral, written, presentation and interpersonal skills.
-computer proficiency-- microsoft word/excel, outlook and internet.
-organized, able to multi-task, think creatively and adapt to different situations.

education and experience:
-minimum education level for lpn: successful completion of az state board of nursing approved licensed practical nurse program.
-preferably a two year degree from an accredited college or university in nursing, public health, or related field.
-preferably three to five years of progressive responsibility in a health care environment; or a combination of education, training and experience that would ensure the effective execution of the duties of the job.
-current arizona state board of nursing licensed practical or registered nurse licensure.
-current fingerprint clearance card or application submitted to department of public safety (dps) within 20 days of hire. we will assist you with this.
-current cpr/first aid certification card from american red cross, american heart association or national safety council. we will assist you with this.
-competency in the position maintained through continuing education

communication skills:
-clear speaking voice, must be fluent in english; bilingual would be a plus.
-ability to speak, work and interface with all levels of co-workers, residents, guests and employees.
-ability to write routine reports and correspondence.
-excellent telephone and customer service etiquette.
