Respiratory support worker developmental post - united kingdom, helston, cornwall
the west cornwall respiratory team have an excited opportunity to recruit a band 3 developmental or experienced band 4 support therapist into their integrated community respiratory team. based at helston community hospital the team consists of respiratory specialists, nurses, physiotherapists, speech and language therapists & dietitian.
this is post will require community working within community venues and patients homes across the west area of cornwall which includes the isles of scilly.
this role is to support care plans initiated by the qualified therapist to optimised management of patients. this is provided through the delivery of education, exercise and specific techniques to improve self-management of long-term respiratory conditions. the team supports patients to live well at home whilst improving quality of life, independence, reducing breathlessness, anxiety, exacerbations, optimising voice, swallow and dietetic needs as well as delivering exercise and education with gold standard, evidence based, pulmonary rehabilitation.the therapy support role is to understand the complexities of mental, physical and social aspects impacting on respiratory health. motivating and linking patients in to support networks within their local communities & other specialist community services as well as supporting discharges from hospital to ensure integration throughout the patient pathway.
