job description:
specific duties:
1.assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
2.identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
3.communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
4. advocate for patients and families as needed to ensure the patients needs and choices are fully represented and supported by the health care team
5.perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating).
must have qualifications/experience:
1. experience with electronic charting
2. high school diploma or ged or equivalent work experience
3. current, unrestricted lpn/lvn license in the state of texas
4. 2+ years of clinical experience in a health care role
5. full covid-19 vaccination is an essential job function of this role.
shift start/end time: monday- friday 8a-5p
dress code: plain black, grey, or navy blue scrubs
are there any specific health screens required?
this position requires tuberculosis screening as well as proof of immunity to measles, mumps, rubella, varicella, tetanus, diphtheria, and pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
