as a community team manager, you will be responsible for maintaining & promoting quality care for our clients and the management of our carers out in the community. you will work towards a set of goals determined by the rm.
you will be required to have a flexible approach to your care duties. you will be required to cover periods of sickness, annual leave and training and to assist with covering shifts which may sometimes be outside your availability. you will be required to complete calls for new packages taken on to enable a thorough and precise service delivery plan to be completed, which is person-centred and outcome-based. you will then complete a handover with the team to ensure that all staff involved in the care delivery are aware of the client's requirements.
main responsibilities
key tasks:
responsible for the quality care provision for clients
to carry out risk assessments and initial service user visits
to ensure ra & care plans are regularly updated and appropriate to the client's needs
to promote outcome-based provision and ensure the client is enabled where appropriate
to ensure the files in the client's folder in their home are always duplicated in the file in the office
to carry out staff supervision and record appropriately in office system
to develop and motivate their team of staff working closely with them and providing support where appropriate
meet with rm regularly and give feedback on client risk assessments, client issues, staff supervisions and staff issues
to be involved with safeguarding meetings, lead investigations and compile reports along with the rm
be part of client case conference meetings with rm and carers involved with the package
on-call duties
attend and contribute to meetings, supervisions and appraisals
promote the image of abicare services ltd and adhere to all requirements set by abicare services ltd standards
general care duties and act as a role model and mentor for all care staff
the ideal candidate
knowledge of cqc regulations
diploma level 3 in health & social care
sound knowledge of 'person-centred' approach to support people
possess excellent organisational, time management and it skills
excellent communication and people management skills
knowledge of care rostering system is an advantage
flexibility to travel regularly to different locations
a clean driving licence and full access to a car are essential
