key responsibilities:

clinical -

assess, plan and evaluate individual packages of care under the guidance/supervision of the team leadercompetently perform clinical skills related to the postassess health needs at individual, practice and community levelunderstand how to use and maintain equipment safely and competently, following appropriate policies and proceduresprofessionally responsible for a delegated caseload under the supervision of the team leadercontribute to the development of clinical guidelinesmaintain professional records in accordance with the trust and nmc record keeping policies.administrative/clerical -

store and maintain clinical records in accordance with trust health records policyinput patient related information onto gp computer systemsreport and manage sickness and other absences to the relevant managerprovide accurate and timely information to the relevant manager regarding annual leave, expenses and study leavesupply required activity data related to patient carecommunication -

communicate with the community nursing team regarding significant changes in patients’ condition which affects patient carecommunication and collaboration with other members of the primary health care team, voluntary and statutory organisations to support individualised patient caresupport the development of integrated ways of working within the phctco-ordinate complex packages of care including attendance at multi-disciplinary case conferences, pre-discharge meetings.managerial -

participate in the planning of shift rotas and allocation of staff under the guidance of or in the absence of the team leadersupport the team leader in the recruitment of junior staffleadership and developmentsupport junior staff and students in their professional developmentcontribute to the public health agendaact as a mentor for pre-registration nursing studentssupport the development of the nursing team by teaching and assessing junior members of the team including health care assistantsteaching of patients / carers and other professionals to support patients to maintain their optimum level of independenceparticipate in pdr process to identify opportunities for enhancing professional or clinical knowledge pertinent the rolemaintain professional portfolio that demonstrates evidence of ongoing professional development and enhancement of skills and competencies, meeting prep requirements as a minimumresearch and audit -

ensure that practice is evidence based through undertaking audit and critically appraising literatureparticipate in audit and researchparticipate in measurement of team’s effectiveness through ongoing monitoring and evaluation of service provision.qualifications and experience:

registered general nurse evidence of recent trainingdemonstrate highest standards and skill in nursing demonstrate working knowledge of community nursing and multi-disciplinary workingknowledge and expertise:

multi agency workingawareness and understanding of the publc health agendaevidence of research based/reflective practice knowledge of local and national priorities ability to manage time effectivley good interpersonal skills analytical and judgemental skills:
