job description:


specific duties:
1. works with the providers and clinic staff to identify patients with chronic disease diagnoses.
2. supports longitudinal care of the patient with stable chronic care conditions
3. supports transition of the patient with stable chronic care conditions from inpatient to outpatient setting
4. notifies providers and/or supervising rn of changes to patients health condition
5. performs accurate and timely documentation in the electronic medical record
6. may perform clinical tasks within their scope of practice


must have qualifications/experience:


1. licensed practical/vocational nurse with an active and unrestricted license to practice in the state of employment
2. current bls certification
3. two or more years of experience in a physicians office, clinical or hospital setting
4. knowledge of chronic diseases, especially copd/asthma, diabetes, chf and ihd
5. excellent verbal and written skills
6. strong interpersonal skills
7. ability to interact productively with individuals and with multidisciplinary teams
8. proficient computer skills to work efficiently with electronic medical records
9. excellent organizational and prioritization skills

shift start/end time: 7:45 am-5:00pm
dress code: teal or charcoal grey scrubs
