looking for a qualified, and swe registered social worker with recent substantive experience of working in a large and busy hospital setting managing discharges and undertaking post discharge care act assessments under the discharge to assess model.candidate must be able to manage competing priorities and a busy workload. candidate must be a good communicator and team player.the candidate will be working within the hospital discharge team that has core operational hours between 830am–5pm, there is some flexibility to work between 830am to 830pm and at the weekend depending on business need.
