Duties and responsibilitiesdisplay specialized knowledge of social service theory, practice, policies, procedures and equipment relevant to pharmacy.demonstrate ability to work in a dynamic environment, which requires critical thinking and advanced analytical and assessment skills.complete required competencies and be responsible for, but not limited to, the following:patient assessment and case management:conduct thorough assessments of patients' needs, including their social, emotional, and physical well-being.develop individualized service plans based on assessment results.coordinate and provide ongoing case management services, which may include referrals to healthcare, housing, education, and counseling.patient advocacy:advocate for patients' rights and access to appropriate services and resources.assist patients in navigating complex systems and procedures.act as a liaison between patients and hospital, government agencies, and other service providers when necessary.offer emotional support and crisis intervention to patients facing difficult circumstances.provide counseling and guidance to help patients overcome personal and social challenges.encourage and facilitate the development of coping skills and self-sufficiency.outreach and education (when necessary):raise awareness of available social services in the community.organize and conduct educational workshops, support groups, and outreach activities to address community needs.provide information and resources to individuals and families to improve their quality of life.documentation and reporting:maintain accurate and up-to-date patient records and case notes.prepare reports on patient progress, service utilization, and outcomes as required.ensure compliance with all legal and ethical standards in record keeping.collaboration and teamwork:collaborate with other social service professionals, agencies, and community organizations to coordinate services.participate in interdisciplinary team meetings and case conferences to ensure comprehensive care.stay current on industry trends and best practices through professional development.ethical and legal compliance:adhere to all ethical standards, regulations, and laws governing social work and patient confidentiality.uphold the highest level of professional integrity in all interactions with patients and colleagues.participates in cqi&ps system control activities, staff development, environmental safety awareness and fire and safety practices, disaster, cpr and infection control.performs other applicable task and duties assigned within the realm of his/her knowledge, skills and abilities.skillsknowledge of community resources, social services, and local support systems.proficiency in computer applications for record keeping and reporting.excellent communication skills in arabic and english.strong knowledge of statistical concepts, methodologies, and software tools.knowledge of relevant industry regulations and compliance requirements.knowledge of the working practices in a major hospital complex.good knowledge and understanding of the patient relation.educationsocial service
