At Creative Minds our mission is to assist our clients be as Independent as possible. At this time our company is looking for Activity Instructors for our Adult Day Program, which work at our facility as well as provide Alternative Services where we work with our clients at home and provide exceptional support.

For DAY PROGRAM Activity Instructors the Essential Duties and Responsibilities are:

Under the direction of the Program Director or supervisor, core duties and responsibilities include the following. Other duties may be assigned.

- Support consumers in meeting the objectives outlined in their individual service plans.

- Review consumers’ initial assessment and individual program plan.

- Provide consumers with the supports necessary to meet their goals and objectives.

- Monitor consumers’ safety in the facility and in the community.

- Teach and develop lesson plans for consumers based on the consumers’ individual service plans.

- Plan and prepare weekly classes and activity schedules including community integration opportunities.

- Plan individual and group activities to stimulate growth in language, social, and motor skills such as learning to listen to instructions, interact and socialize with others, and using activity equipment.

- Provide data collection related to individual consumers that reflect progress, problems, solutions and other pertinent data by maintaining and updating the monthly progress logs and daily case notes.

- Attend initial orientation, all in-service trainings and staff meetings.

- Keep working areas clean and organized.

- Maintain all equipment and materials assigned.

- Transport consumers to activities (as designated).

- Assist consumers with meals and refreshments during their lunch and as needed in the facility and during community outings.

For our ILS/SLS Instructors the Essential Duties and Responsibilities are:

Under the direction of the ILS / SLS Director, Program Manager core duties and responsibilities include, but are not limited to the following:

- Review Creative Minds Initial Assessment and Individualized Support Plan for each assigned consumer

- Provide support, instruction, assistance and monitoring to address the goals and services outlined on the consumers’ service plan and goals

- Monitor consumers’ safety in their home and community

- Reporting: Special Incident Reports (SIR), Adult Protective Services reports (APS), police reports (when applicable), other necessary reports when requested

- On call response as assigned outside of typical program office hours for client emergencies and support

- Meet with program administrative staff to provide data related to assigned consumers, to create and compose semi-annual and annual progress reports submitted to funding sources

- Attend and participate in initial training orientation, all in service events, and mandatory staff meetings and trainings

- Be available to participate in program events and activities

- Be prepared and available with all materials prior to providing services

- Complete and submit all required program documentation when required (Case notes, Timesheets, progress updates, etc.)

- Maintain all equipment and materials assigned by management

- Transport consumers to activities as outlined in their service plan

- Respond and assist in emergency situations when required, administer first aid and CPR if necessary, notify management

- Able to work a flexible schedule to include weekends and holidays

- Be available for additional responsibilities and assignments at the request of management

- Performs any other job-related duties as assigned by Supervisor.

- Acts as a representative of Creative Minds and communicates with the service recipient, family, outside agencies, and community entities. Provides services in accordance with the spirit and intent of the independent living and Creative Minds philosophy.

- This position has no assigned supervisory responsibilities.

Both Positions Require:

· Must have a Valid California driver's license

· Must pass pre-employment and maintain a clean criminal background screening / fingerprint clearance while employed

· Must pass health screening and TB testing

· First aid and CPR certification (training is provided by employer)
